Effect of Educational Training Program on Improving Drug Adherence among Adolescents with Epilepsy

Abstract Background: Epilepsy is a chronic medical condition, which requires long term management. Adherence to antiepileptic drugs is crucial for preventing the risk of recurrent seizures.The consequence of non-adherence can severely compromises patient health, quality of life and survival. Aims: was to evaluate the effect of educational training program on improving drug adherence among adolescents with epilepsy. Subjects & methods: Research design: A quasi experimental design was used. Subjects: A purposive sample of 50 epileptic adolescents. Setting: Pediatric Neurology Outpatient Clinic at Zagazig University Hospital, Zagazig, Egypt. Tools of data collection: three tools were used to collect the necessary data included; a structured interview questionnaire, Epilepsy knowledge questionnaire,Morisky 8-Item Medication Adherence Scale (MMAS-8) and number of seizure frequency. Results: It was found that epileptic adolescents' adherence to antiepileptic drug therapy and knowledge about medication were significantly improved after implementation of the intervention (P=0.001). Conclusion: the educational training intervention has a profound effect on improving adherence to antiepileptic drug therapy, knowledge about disease and medication with dramatically decline in seizure frequency. Recommendations: drug adherence strategies must be discussed with epileptic adolescents and their families to help them comply with medication regimen.

Guiding Program for Mothers to Enhance Home Care and Children’s Health Outcomes after Cardiac Surger

Background: Serious problems can occur after cardiac surgery, so mothers need more comprehensive home care educational programs to help them recognize complications and care for their children following hospital discharge. The Aim of the study was to evaluate the effect of a guiding program for mothers on enhancing home care and the health outcomes of their children after cardiac surgery. A quasi-experimental design was used to conduct the present study at the Cardiothoracic Surgery Department and Pediatric Outpatient Clinic at Zagazig University Hospitals. Subjects: A sample of 48 mothers and their children scheduled for cardiac surgery at the previous settings. Tools: Three tools were used to collect data. A Structured Interview Questionnaire, Reported practice checklist and Children’ assessment sheet. Results: Statistically significant differences regarding mothers’ knowledge and reported practice were observed at the post-program and follow-up phases, with a statistically significant strong positive correlation between them. Statistically significant improvements also revealed regarding children’ health outcomes post program. Conclusion: The guiding program had a profound effect on enhancing mothers knowledge and practice about home care as well as children’s health outcomes after cardiac surgery. The study recommended that educating mothers regarding the care of their children after cardiac surgery should become a routine practice at the cardiothoracic surgery department. Assessment of Maternal Care Provided To Their Epileptic Children At Zagazig University Hospitals

Background: Epilepsy is the commonest serious neurological condition of childhood. It is a group of chronic disorders in which the indispensable feature is recurrence of seizure. Parents, especially mothers play the most significant role in caring for their children and helping them adapt to their conditions. Aim of the study was to assess maternal care provided to their epileptic children at Zagazig University Hospitals. Subjects & methods: Research design: A descriptive – cross sectional design was used. Setting: at the Pediatric neurology outpatient clinic at Zagazig University Hospital. Subject: 120 mothers of children with epilepsy.. Tools of data collection: three tools were used in this study; the first was a questionnaire interview sheet to collect data about studied mothers and their children. The second was seizure severity scale. The third tool was a questionnaire sheet about mothers' knowledge regarding practices in caring for their epileptic children.Results indicated that the mean age of the studied mothers was 33.3 ± 6.4 years. The majority of them were housewives and 39.2% of them had diploma education. It was showed that, 59.2% of them did nothing at the beginning of fit. During an epileptic fit, 37.5% of mothers did nothing while, 44.2% of them mentioned unfavorable practices such as, stimulating the child by pouring water and beating, restricting or shacking the child to awake him/her up. As regards care practices after fit, 57.5% of mothers did nothing. Moreover, it was found that 67.5% of mothers didn't take any action to manage side effects of antiepileptic drugs. About 51.6% of epileptic children had severed degree of seizure. Conclusions: most of the studied mothers had poor practices score in caring for their children with epilepsy and only 11.7% of them had fair practices. Based on the results of the present study, it could be recommended that educational programs for mothers of epileptic children about epilepsy and its care should be conducted

Adherence to GFD, knowledge, and health-related quality of life of children with celiac disease.

Background Living gluten-free is a well-known concept for children with celiac disease who need to be knowledgeable and closely adherent to a gluten-free diet (GFD) to ensure good health-related quality of life. The study was conducted to assess adherence to GFD, knowledge, and health-related quality of life of children with celiac disease. Subjects and Methods a descriptive cross-sectional study was conducted on 103 children with celiac disease at the outpatient clinic of Gastroenterology and the Bakery for Special Therapeutic Nutrition at Zagazig University Hospitals. Data collection tools included; Leffler’s questionnaire for adherence, GFD knowledge quiz, and celiac disease-specific pediatric health-related quality of life (CDPQOL). Results Less than one-fifth of the studied celiac children reported excellent adherence to GFD and less than one-sixth of them had good knowledge about GFD. Low total health-related quality of life scores were found in about three-fifths of children aged 8-12 years and nearly half of adolescents aged 13-18 years. Furthermore, adherence to GFD showed a significant positive correlation with health-related quality of life. Conclusions Children with celiac disease had poor knowledge about GFD and poorly adhered to it. They also reported low health-related quality of life that was affected by poor adherence to GFD. Recommendations Continuous evaluation of children's adherence to GFD and discussion of children’s beliefs and concerns about dietary regimens should be done routinely.
